For customers who need their handset repaired but cannot get to a store, please download this form and take it along with your handset to the service center.

Direct debit form

To pay your bills by Direct Debit^ from your nominated financial institution you will need to:

  • Update your Direct Debit details via My3 Online.
  • Download the Direct Debit Form and fax to 1300 763 113 or mail to PO Box 202 Spring Hill, Qld 4004.

^ Credit/debit cards are subject to a surcharge fee of 1% (plus GST) for Visa, Mastercard & Diners and 2.15% (plus GST) for Amex of the total transaction amount . This will appear on your next bill.

Undo an unauthorised Number Port

Need to manage a reversal of unauthorised port? Download the MNP Reversal Authorisation Form

To request a port reversal where a number is considered to be incorrectly ported out of 3. We will contact you within 2 working days with the status of your request.
